I started in the era when there was only design and development. And Design included HTML+CSS, as developers had “real things” to do ;-)
It’s all a problem of large organizations and smaller ones trying to replicate the processes of the large one.
At the end everyone pats everyone else on their back at another UX conference and congratulate on a “job” well done — 3 months of card sorting and persona building. Then the final product has an ad that’s covering the checkout process ;-) Yay!
